Circulatory Failure (NC) – it is a pathological condition in which the blood to the organs of the human body in the right quantity not delivered. Due to a malfunction in the cardiovascular system the human body cannot properly function. This disease is found quite often: every hundredth inhabitant of our country suffering from such a syndrome. Medical statistics show that the annual increase in the incidence of 3 cases per 1000 people. Each year in steady-state conditions are treated only 35% of people suffering heart failure.

Classification of disease

Distinguish the syndrome of heart failure associated with heart disease and a disease associated with circulatory disorders of the cardiovascular system. Classic clinical manifestations of vascular failure is the occurrence of collapse, orthostatic hypotension, and shock.

Types of failure

The Classification of diseases according to their flow is divided into acute and chronic. The concept of acute insufficiency applies to the designation of cardiac asthma and pulmonary edema. The acute form usually develops rapidly in the background of myocardial infarction, rupture of the walls of the left ventricle, myocarditis, or acute failure of the aortic and mitral valves, prolonged circulatory failure or other pathologies. With the development of acute forms occur adverse clinical signs of disease and the possible prognosis of fatal outcome.

With the gradual development of failure there are chronic form. It can be formed from several weeks to tens of years. The reasons for its occurrence may be the presence of heart disease, hypertension, chronic respiratory failure or prolonged anemia. Today there are two classifications of circulatory failure. The first was proposed about a century ago. It is based on a functional approach and a qualitative assessment of changes occurring in the body.classification failure

Classification by N. D. Strazhesko and V. G. Vasilenko

Suggested this method of separation – outstanding Soviet cardiologists-clinicians Strazhesko and Vasilenko. Classification of circulatory deficiency is identifies three different stages of the disease.

The first, primary and most often latent stage (I), in a patient at rest, symptoms with any violations are observed. Only during exercise can occur shortness of breath, fatigue or palpitations. This can be signs of compensatory hypertrophy and/or dilatation tonogennoy one or the other chamber of the heart.

The Second stage (II) according to the classification of heart failure is divided into two subgroups in which manifestations of the disease will have more pronounced symptoms.

The Second stage of the first podstudio (II-A) is failure of the right or left heart, the phenomena of stagnation, mild expressed and quickly passing after proper treatment and compliance with the regime. Shortness of breath and weakness are observed at rest, during exercise they increase considerably, causing full-scale operations impossible. There are metabolic disorders and disruptions in the activity of certain organs. The liver can be a bit more normal size and painful, but in reasonable limits. By evening, feet might swell, the swelling goes away.

According to the proposed classification, the second stage of the second podstudio (II-B) is characterized by signs of blood stagnation in small and big circles of blood circulation, which is expressed alone. Even a slight increase in physical activity leads to increasing shortness of breath. There is a lack and right and left heart. There are signs of ascites and diffuse cyanosis. The liver is enlarged and painful disorders in the kidneys, and edema.

The third, final, dystrophic stage (III) – irreversible disturbances in organs and systems, and are more likely to suffer liver and lungs.

Offer American cardiologists

However, to date, the best classification of heart failure severity is the offer of the new York heart Association (NYHA). According to the American classification, heart failure can be subdivided into functional classes, each of which depends on the physical load that can be absorbed by the patient. It is based on the principle of degree of pathology of hemodynamic disorders in a large and small circle of blood circulation. Thus, the American proposal allows cardiologists to determine the health of a patient or the ability to carry out any activity without the appearance of complaints that are specific for heart failure.

Functional classes

Classification of heart failure functional classes do not necessarily correspond to certain stages of the manifestation of failure. Depending on the treatment therapy and the degree of progress of the disease, functional class may change.

According to the level of efficiency and degree of tolerance to patients of physical activity, chronic heart failure (CHF) is divided into four classes:

  • First CHF functional class. These include physically active patients. From their side there are no complaints of shortness of breath or palpitations with increasing loads.
  • Second with CHF functional class. Patients, which are characterized by moderate limitation of physical activity. The patient feels at rest, however, even the usual physical exertion causes fatigue, shortness of breath or tachycardia.
  • Third with CHF functional class. Patients with a pronounced character of the disease. Even when at rest, the patient feels weakness, shortness of breath and heart palpitations.
  • Third with CHF functional class. Discomfort may occur even with the slightest exercise. Even motionless, the patient is experiencing symptoms of deficiency. The activity of such patients is minimized.

Classification of chronic circulatory failure in the American system is quite simple and convenient. That is why it is common in international and European societies of cardiology.

Why there is chronic heart failure?

Heart failure most often occurs due to destruction of the heart muscle or loss of ability to pump the required amount of blood through the vascular system. The major causes of this disease are:

  • High blood pressure.
  • Coronary heart disease, myocardial infarction, myocarditis, education cardiomyopathy or atrial fibrillation, heart block.
  • Changes in the muscle structure that occurs as a result of taking some drugs.
  • Diabetes mellitus, disorders of the kidneys or liver.
  • Congenital malformations.
  • The Presence of harmful habits such as Smoking, use of alcohol or drugs.

The Classic symptoms

Depending on the degree and type of violation can manifest different symptoms. If the failure occurred in the left ventricle of the heart, stagnation of blood exposed to the pulmonary circulation and pulmonary system. In this case, the patient is experiencing shortness of breath, signs of cyanosis (blue lips, fingers and toes), there are dry rales, possible hemoptysis.

If the affected right ventricle, the blood stasis is exposed to the systemic circulation. In this case characteristic: swelling of the limbs and pain in the right hypochondrium (the stagnation of blood in the liver).

In any case, the patient experiences fatigue and palpitations. In the classification of circulatory failure in children or adults, symptoms will be more intense, if the disease became more severe.

Diagnosis

Heart failure is determined on the basis of examination of cardiologist and the use of additional means and methods of the survey. The state of cardiac activity is evaluated using ECG monitoring during the day and using the treadmill tests. Echocardiogram allows to set the level of contractility and dimensions of the various parts of the heart. To determine the extent and degree of blockage of blood vessels, as well as to explore the level of pressure in the chambers of the heart, allows such manipulation as cardiac catheterization.circulatory failure classification according to severity

Treatment drugs

As the main means of treatment of circulatory failure, classification of which was given above, are:

  • Beta-blockers (inhibitors). These medicines help to reduce blood pressure and decrease heart rate.
  • Glycosides. These drugs are effective in addressing the main symptom...
